Add a post-purchase survey or form to the Thank you page

Show a post-purchase survey or Customer Fields form to signed-in customers right after checkout.

A post-purchase survey or form asks customers for information right after they order. In Customer Fields, you build one with a Post-checkout form, which shows one of your forms on Shopify's Thank you page. Use it for a quick survey, like "How did you hear about us?", or for details you don't need at sign-up, like a birthday or preferences. Answers save to the customer's metafields, the same as any other Customer Fields form.

Before you start

  • The form only shows to customers who are signed in when they check out. To make sure every customer sees it, turn on Require customers to sign in to their account before checkout in Settings > Checkout in your Shopify admin.

  • You need an Account/Checkout form. If that option shows a lock, your plan doesn't include custom fields. Upgrade, or contact support.

  • If your store still uses the legacy bundled JSON metafield storage method, switch to the Shopify admin-compatible storage method in the app's settings first. Account/Checkout forms don't work with bundled storage.

Create the form

  1. In Customer Fields, go to Forms > Create form and choose Account/Checkout.

  2. Add your custom fields, and any headings or paragraphs you want. Forms can have more than one step.

  3. Save the form.

Choose how often it shows

In the form's settings, the Post-checkout section has one option: Hide if previously submitted. It's on by default, so a customer who has already filled in the form won't see it on later orders. Turn it off to show the form after every order.

The submit button says Save by default. To change it, for example to "Submit", edit Save in the form's Language settings.

Add it to the Thank you page

  1. Open the form's Install form page and copy the form ID. It's six characters long.

  2. In your Shopify admin, go to Settings > Checkout and click Edit on your checkout configuration. In the page menu at the top, choose Thank you.

  3. Add the Post-checkout form app block from Customer Fields.

  4. Paste the form ID into Form ID.

  5. Optionally, set a Success message of up to 30 characters. If you leave it empty, customers see "Thanks! Your response has been submitted."

  6. Save in the editor, and publish if you're working on a draft.

What happens when a customer submits

  • Answers save to the customer's metafields and show on the customer in Customer Fields and in the Shopify admin.

  • Form rules run, so you can tag customers based on their answers.

  • Shopify Flow's Customer submitted form trigger runs. See How to use Customer Fields with Shopify Flow.

Limits

Guests

The form doesn't show for guest checkout. Only signed-in customers see it.

Order data

Answers save to the customer, not the order. The form can't read order details, like what was bought.

Page

The block shows on the Thank you page only, not the Order status page.

Form type

Only Account/Checkout forms work. With a storefront form's ID, the editor shows an error and customers see nothing.

Troubleshooting

The form doesn't show after an order. Check that the customer was signed in when they checked out, and that they haven't already submitted the form while Hide if previously submitted is on.

The editor shows "Form ID does not exist or belongs to an archived form." Check the ID on the form's Install form page, and that the form isn't archived.

The editor shows "This extension can only be used with account/checkout forms." The ID belongs to a storefront form. Use an Account/Checkout form instead.
