A post-purchase survey asks customers a question right after they order, while the purchase is fresh. Asking "How did you hear about us?" tells you which marketing brings in buyers, so you know where to spend. In Customer Fields, you run it with a Post-checkout form on the Thank you page, and each answer saves on the customer.
Who this works for
The form only shows to customers who are signed in when they check out. It's a good fit if your store already requires customers to sign in before checkout, for example a wholesale or members-only store. To require it, turn on Require customers to sign in to their account before checkout in Settings > Checkout in your Shopify admin.
If most of your customers check out as guests, the survey will miss them.
Build the post-purchase survey
Go to Forms > Create form and choose Account/Checkout.
Add a Dropdown field, for example "How did you hear about us?" with options like Instagram, YouTube, a friend, search, or a podcast.
Add an "Other" option and a short text field for customers to explain. Use a form rule with the show field or element action to show the text field only when they pick "Other".
Keep it short. One or two questions get the most answers.
In the form's Language settings, change Save to something like "Submit".
Leave Hide if previously submitted on, so you ask each customer once. Then add the form to the Thank you page. See Add a post-purchase survey or form to the Thank you page for each step.
Use the answers
See them in Customer Fields | On the Customers page, use Add filter and choose the answer's data column, or export to a spreadsheet. See Export customer data to CSV. |
Tag customers | Use a form rule with the add tag to customer action, for example |
Automate with Shopify Flow | Start a workflow with the Customer submitted form trigger, for example to post new answers to Slack. See How to use Customer Fields with Shopify Flow. |
Email by source | Tag customers by their answer with a form rule (above). Klaviyo's Shopify integration syncs customer tags, so you can build a Klaviyo segment from the tag. |
Good to know
Answers save to the customer, not the order, so you can't see which order a customer answered on.
Each customer has one answer. If you turn off Hide if previously submitted, a new answer replaces the old one.
